My HealtheVet is the VA's Personal Health Record. It was designed for Veterans, active duty service members, their dependents and caregivers. My HealtheVet helps you partner with your health care team. It provides you opportunities and tools to make informed decisions. Specific features in My HealtheVet are available to you based on your account type. All users who have a Basic account are able to view their self-entered information. If you are a VA patient, you can upgrade your account to Advanced or Premium*. These account types may allow you to refill your VA prescriptions online and view parts of your VA health record and/or DoD Military Service Information. Alcohol and prescription drug abuse are higher among active duty service members and veterans. Many of these people suffer from underlying post-traumatic stress disorder (PTSD).

Veterans Inc. is a national leader in ending homelessness among veterans. We have one of the highest rates in the nation (85%) for transitioning veterans out of homelessness. Headquartered in Massachusetts, Veterans Inc. is the largest provider of services to veterans and their families in New England. Since 1991, we have helped more than 40,000 veterans and their families. Operations are expanding further into New England and beyond in our goal of eradicating homelessness among veterans.

Our Mission Now in its third decade of service, The New England Center for Homeless Veterans (NECHV) is a multi-dimension service and care provider dedicated to assisting Veterans who are facing or at-risk of homelessness. The Center offers a broad array of programs and services that enable success, reintegration, meaningful employment and independent living.

We provide shelter and services, with a special focus on healing from substance abuse and learning new life skills. The VTH is a 501 (c) (3) non-profit , founded in 1990 by a group of Vietnam veterans and community leaders who recognized the vital need for housing and supportive services for area homeless and at-risk veterans�??especially those tackling substance abuse�??on the Massachusetts South Coast. Our goal is the rehabilitation, self-sufficiency and reintegration of each veteran into the community. It's our belief that the veterans we serve seek a hand up, not a hand-out.

Homeless shelter dedicated to veterans located in Manchester, NH. For more than a decade, Liberty House in Manchester, NH has been providing a safe, supportive, substance-free housing community for American veterans transitioning out of homelessness. Since opening our doors in 2004, our dedicated team of staff and volunteers has helped more than 200 homeless veterans rejoin their communities and regain fulfilling, independent lives. Each and every veteran that walks through our doors is treated with the utmost respect, regardless of their combat status or length of service, and provided with a unique transition plan tailored to their needs. With the guidance and support of brother and sister veterans and a commitment to giving back through food pantry and clothing programs, Liberty House is more than just a community; it's a family. Our transitional facility offers ten resident beds and a variety of services ranging from employment and housing assistance to case management and agency referrals.

Operation Stand Down Rhode Island (OSDRI) is an independent 501(c)(3) non-profit organization and Rhode Island�??s primary nonprofit resource for homeless and at-risk veterans. Founded in 1993 and headquartered in Johnston, OSDRI�??s mission is to help homeless and at-risk veterans secure stable housing. Ending the cycle of veteran homelessness, and preventing it, requires much more than providing a roof over the head of an individual veteran or veteran family. To achieve this goal, OSDRI offers a wide range of life-changing services including supportive permanent and transitional housing, intensive case management, basic human needs assistance (for example, food and clothing) and an employment training and placement program. OSDRI works hand in hand with almost every major state and federal agency as well as community organizations and employers to provide seamless referrals to outside sources of assistance.

Soldier On is a private nonprofit organization committed to ending veteran homelessness. Since 1994, the organization has been providing homeless veterans with transitional housing and supportive services. In 2010, Soldier On opened the first Gordon H. Mansfield Veterans Community, a permanent housing cooperative that provides formerly homeless veterans with safe, sustainable, affordable housing �?? transitioning them from homelessness to home-ownership. This housing model will be replicated nationally as Soldier On continues to change the end of the story for homeless veterans throughout the country.

Harbor Homes is a non-profit community-benefit organization that provides low-income, homeless, and disabled New Hampshire community members with affordable housing, primary and behavioral health care, employment and job training, and supportive services. As a member of the Partnership for Successful Living, Harbor Homes offers a unique, holistic approach to care that results in better outcomes for our clients and the community.

Ending Homelessness One Life at a Time Our philosophy is Housing First. We know that getting someone into a safe, secure place is often the key to success in other areas of life. We have a 90% success rate with those in supportive housing, a situation in which individuals or families have a case manager in their first year of FBMS sponsored housing.

Habitat P.L.U.S has two program sites located right next to each other. The first site is the group home or as it is often referred to as the main house. The second site is the cooperative apartment program.
